Listen to patients and respond to their concerns
• Administer medication as prescribed by the patients Physician
• Provide palliative care as needed to keep the patient comfortable
• Performs evaluation tasks, including vital signs and medication review
• Monitors patient recovery and compile reports for the attending Physician
• Assess patient’s condition during every visit and chart every observations
• Coordinate with Therapists or Physicians and anyone involved in the patients care plan
• Educate patients and their families on proper home healthcare strategies and procedures
• Provide suggestions to the Physician or family members to improve the health of the patient
